The Shared Transfer of Care eLearning course is a free online course.  It is designed to support staff in the health and community services sector working with people who are being discharged, referred, or transferred to other services.  The course takes around three to four hours to complete. 

Consumers with chronic and complex health needs can experience multiple transfers across the health sector, leading to a higher risk of adverse events, with an impact on health outcomes and a reduction in wellbeing and trust in the system.

The course supports participants to acquire the knowledge and skills to implement quality shared transfers of care. A certificate will be issued once the course is completed.

While there are tools throughout this course which may be useful to print out and use in your workplace, the course itself is designed to be taken online as it contains multimedia and interactive elements.

On completion of the course you will have gained knowledge described in each of the 7 Learning Objectives (LO) below:

LO 1: Person-Centred Care

You will be able to describe the advantages in health outcomes of person-centred care compared with practitioner-centred care, including the importance of a self-management approach.

On completion, you will be able to demonstrate strategies for delivering a person- centred approach in interactions with health service users and other health professionals.

LO 2: Accountability in the transfer of care process

You will be able to explain the importance of, and processes for, shared accountability in the transfer of care.

On completion, you will be able to demonstrate how you can use an accountable process to deliver transfer of care.

LO 3: Effective shared communication between service providers and consumers

You will be able to describe effective communication (i.e. health literacy) with consumers, and identify strategies and tools that can help mitigate communication barriers.

On completion, you will be able to demonstrate, through participation in scenarios, how to effectively share information with consumers.

LO 4: Effective shared documentation between health care providers, and between providers and consumers

You will be able to describe why effective and timely shared communication and documentation between health care providers, as well as between providers and consumers is essential to ensure safe transfer of care.

On completion, you will be able to demonstrate, through reflective scenarios, your role in facilitating clear and accurate flow of information to service providers at every transfer of care point.

LO 5: Enablers and barriers to shared transfers of care

You will be able to describe the transfer of care points for the older person's health journey and the barriers and enablers for a quality transfer of care.

On completion, you will be able to demonstrate what is required to support a safe transfer of care at the various transfer points.

LO 6: Effectively coordinating care

You will be able to outline the principles of coordination of care and their role in quality transfers of care, including defining the roles of service providers within transfers of care.

On completion, you will be able to demonstrate, through scenario-based planning, your role in coordinating care and that of other service providers.

LO 7: Applying the Shared Transfer of Care Sharing Points in practice

You will be able to list the 5 Shared Transfer of Care Sharing Points and explain their meaning.

On completion, you will be able to demonstrate how you would apply the Sharing Points in practice using a strategic plan approach.
