Primary Health Tasmania has produced a wide range of resources for healthcare professionals to:

  • improve transfers of care between acute, primary and community services sectors for people living with chronic complex needs
  • focus on capacity-building and system redesign to improve transfers of care and reduce avoidable hospital readmissions
  • improve or ‘streamline’ existing practices and processes within the health system.

Guidelines for Shared Transfer of Care

The Guidelines for Shared Transfer of Care have been developed to provide direction to health professionals working across the health care sector on improving transfers of care. The Guidelines document sets out and describes the five Sharing Points which are key elements for delivering best practice transfers of care.

Passport to Better Health

The Passport to Better Health is a booklet aimed at giving consumers a more active role in sharing in their care. It allows consumers to track information related to their healthcare journey, including personal details, health conditions, appointments and medication.

It also gives consumers a platform to communicate with care providers about personal preferences, concerns, needs and questions about their health and wellbeing.

The Passport to Better Health has been developed in consultation with consumers across Tasmania to increase relevancy, usability and accessibility.

ISOBAR SHARED Planning Checklist

The ISOBAR SHARED Planning Checklist is intended for use by clinicians across sectors. ISOBAR is a clinical communication framework that supports sharing of quality communication and documentation.

SHARED is the person centred approach that supports the ISOBAR framework. The tool can be used as a poster, resource or prompt in file to support quality shared transfer of care, or as an education tool for students undertaking study in the health and community sector.
